Employment Eligibility

Use the Employment Eligibility form to identify the documents the employee has provided to prove entitlement to work legally in the country. The items listed in the I-9 List B generally prove that the person's identity is as represented, the items listed in List C prove that the person is authorized to work in the country, and the items in List A generally prove both requirements.

In the Document 1 group box, enter information for the first document the employee has provided as proof of eligibility to work in the country.

In the Document 2 group box, enter information for the second document the employee has provided as proof of identity or of eligibility to work in the country.

Note: The Document 2 fields are not available if the Document 1 I-9 List field shows the required proof of work eligibility and identity included the one item required from List A.
