Facility

Name of the destination issuing demand to which parts are to be sent.

Ship To

Name of the shipping destination to which parts are to be sent.

Part Number

Part number as established by the customer.

Starting Decision Date

The date range on which to base the search.

Ending Decision Date

The date range on which to base the search.

Decision

Whether or not the part meets requirements. Full Approval indicates that the part successfully meets all requirements. Provisional Approval indicates that modifications must be made to the part, but it has been tentatively accepted. Rejected indicates that the test part is unacceptable.
