Completing the unit questionnaire

This topic describes how to complete the unit questionnaire.

Note: Accessing the questionnaire through the Admin Portal is recommended for the best user experience. However, the Self-Service Portal can be used if access through the user's phone is necessary.
  1. From the Admin Portal, select Clinical Science > Unit Questionnaire.
    From the Self-Service Portal, select Clinical Science > Unit Questionnaire.
  2. On the Ranked Questions page, select the Unit, Day and Day Part for the shift. You must select a shift from the past.
    If the message, "No Unit Questionnaire Available" displays, it means that no assignment information is available for the shift and the questionnaire cannot be run.
  3. For each question that displays on the Ranked Questions page, select the appropriate response: Adequate, Inadequate, More than Adequate. Click Next when you finish responding to a question.
  4. If you selected Inadequate for any question, the Staffing Factors page displays. You must select at least one reason that describes the inadequate situation. You can select multiple reasons. If you do not provide at least one response from the list of reasons, and attempt to click Next, an error message results.
    If you selected Adequate or More than Adequate for all ranked questions, the Staffing Factors page is not displayed.
  5. Optionally, on the Shift Comments page, add notes about the shift. Click Next to move to the next page.
  6. On the Staffing Hours Assessment page, review the data that is populated for the shift for each position or job (RN, support employee, and so on). If the Scheduled Hours column adequately reflects the number of hours that were required for each employee type on the shift, do nothing. If the number of hours is too low or too high, provide the number of hours that were actually required.
    For example, if the shift was staffed for 40 RN hours but 48 were needed, enter "8" in the "+/" column. If the shift was staffed for 40 RN hours but only 32 were needed, specify "-8" in the "+/" column.
  7. Click Submit when you are finished reviewing the Staffing Hours Assessment page.
    You are presented with your responses in view-only mode. If you want to make a change, click the pencil icon on the top of the Ranked Questions page to begin making changes. Click Submit again when you complete your changes.
