| PPAP ID | The code of the PPAP report provided by the customer. |
| Facility | The name of the location from where the demand for the parts is raised. |
| Ship To | The location to which the parts are shipped. |
| Part | The part for which the PPAP report is generated. |
| Decision Date | The date on which the decision is made regarding the part meeting the customer requirement. |
| Decision | Indicates the decision with regards to the
