Adding or modifying the survey questions

Use this page to add or modify the survey questions.

  1. Select Surveys > Surveys Setup. The List of Surveys page is displayed.
  2. Click Edit corresponding to the Survey Name to which the questions must be added. The Add/Edit Survey page
  3. Click Questions. The List of Questions page is displayed.
  4. Click Add. The Add/Edit Survey Question page is displayed.
  5. Specify this information:
    Sequence No
    The serial number of the question. This serial number is generated automatically.
    Question
    The question related to the survey.
    Help Text
    The additional information related to the survey question.
    Response Type
    The format of the survey response. Possible values:
    • Text
    • Numbers
    • Decimals
    • Yes/No
    • Dropdown
    • Multiselect
    • Date
    Response Option
    The text box to specify the possible values related to the survey question. This is displayed only when you set the response type to Text, Dropdown and Multiselect.
    Suggested Response
    The response for the survey question.
    Mandatory
    Select this check box to mandate the survey question.
    Importance
    The priority set to the survey question. Possible values:
    • Critical
    • Major
    • Minor
  6. Click Save and Add to add the next survey question.
  7. Click Save and Return to return to the List of Questions page.
