Specify Form Template Name dialog box
Use this dialog box to enter a name for the form template being created from the current form.

| Name | This field specifies the name for the new form template, which must be unique.
By default, the form name for the current form is displayed. |
| Existing Form Templates | This field displays a list of existing form templates that are stored in the templates database. |
