CB221 - Shows transactions specified by different users as having the same operator ID

CB221 Bank Transaction Listing report is modified to use the new CREATED-BY field as Operator Id for CB10 Cash Deposit transactions. Note that for unreleased transactions from CB15.1 and CB15.2, the operator ID is displayed as blank.
