Processing the EEO-5 report

The EEOC notifies employers who are required to submit the EEO-5 Survey report by direct mail to the school system or district, or by indirect mail to the school system or district through the State Education Agency. The selected schools and districts run the EEO-5 Survey Report to gather the required statistics as of payroll periods that includes October 1 (e.g., October 1, 2012). Selected schools and districts must file the EEO-5 Survey Report to the address indicated on the notification letter by November 30th (e.g., November 30, 2012).

To create the EEO-5 report

  1. Access EEO-5 Report (PA392).
  2. In the Parameters section, company field, select the company. This field is required.
  3. In the Main tab, use the following guidelines to enter field values:
    Dates of Payroll Period Used

    Selected schools and districts run the EEO-5 Survey Report to gather the required statistics as of payroll periods that includes October 1 (e.g., October 1, 2010). These are required fields.

    Date Option

    Select which employee date field (Hire Date, Adjusted Hire Date, or both) you want to use to compare to the Dates of Payroll Period Used to determine employment. This is a required field; defaults to the employee Hire Date.

    Check History

    Select whether or not you want to check employee job history during the creation of the report. This is a required field; defaults to No.

  4. In the Statistics tab, enter statistics specific to your EEO-5 filing; these fields are not required.
  5. In the Status tab, categorize Employee Status Codes (HR03.1) values into full-time and part-time designations.
  6. In the Ethnicity tab, categorize Ethnicity (HR91.1) values into EEO-5 reporting classifications. You must enter at least one ethnicity for each report classification.
  7. In the Electronic File tab, designate whether or not to create file for electronic processing in addition to generating the report. This is a required field; defaults to No. The create file is only valid for a Single Establishment. EEOC will only accept a single record in the file. The Multi-Establishment option would create multiple records in the file.
