Generating Benefit Enrollment Forms

This procedure enables you to run a report to create benefit election and confirmation forms. This report displays the plans in which an employee is eligible, the plans which an employee is enrolled in or both.

To generate benefit enrollment forms

  1. Access Benefit Election Form (BN245).
  2. Complete the information in the Parameters section.
  3. If you want to create a data file so you can use to export this information into an external software product, select either Both Report and CSV or CSV Only in the Report Option field.
  4. Select the Add function.
  5. Submit the report.
