Form Wizard, Extend Form page
Use this page to provide the specifications to create a new form by extending an existing form.
On this page, you must provide the name for the new form and specify the existing form on which the new form is to be based.
When you do this, Windows client automatically provides a recommended Description, which you can modify.
To load the new form when a request to open the base form is received, select the check box.
