Provide the title of the notice.

Provide an explanation for the product that is recalled.

Type

Specify a customized type to categorize notices.

Date Of Notice

Specify the date when the notice is identified and submitted.
- Click Next.
- On the Product Information page, click Create and specify the product information.
- Click Next.
- On the Attachments page, click Create to include files in the report.
- Click Next.
- On the Summary page, click to view the notice details.
