Reporting a notice

  1. Select Manage Notices > Report A Notice.
  2. On the Notice Information page, specify this information:
    Name Of Notice
    Provide the title of the notice.
    Description Of Notice
    Provide an explanation for the product that is recalled.
    Type
    Specify a customized type to categorize notices.
    Date Of Notice
    Specify the date when the notice is identified and submitted.
  3. Click Next.
  4. On the Product Information page, click Create and specify the product information.
  5. Click Next.
  6. On the Attachments page, click Create to include files in the report.
  7. Click Next.
  8. On the Summary page, click View Notice to view the notice details.
