Creating a form

  1. In Form Builder, select Builder.
  2. Click Create New.
  3. In Properties, specify this information:
    Form Name
    Specify the name or title of the form. This field is required.
    Description
    Provide a description for the form. This field is optional.
  4. To add a confirmation message upon the submission of the form, select the title of the message from the Submit Message dropdown menu.
  5. Click Save.
  6. To assign a group that can access the form:
    1. In Groups, select Unassigned Groups.
    2. Select a check box corresponding to a group. You can select multiple user groups.
    3. Click Assign Group.
  7. To remove the access of a user group to the form:
    1. In Groups, select Assigned Groups.
    2. Select a check box corresponding to a user group to remove form access. You can select multiple user groups.
    3. Click Unassign Group.
  8. To add fields to the form:
    1. Select the Fields tab.
    2. Click Add.
    3. In the Field Type, select from these field types and specify the required field details:
      • To add a Plain Text, specify this information:
        Field Text
        Specify a text that shows static, non-editable content on the form. Use this for instructions, labels, or section headers.
        Order
        Specify a number to define the position of the field type on the form.
      • To add a Dropdown, specify this information:
        Field Label
        Specify a name for the field. This is displayed on the form.
        Dropdown Name
        Specify the name of an existing dropdown option.
        Default Value
        Select the value that shows in the field when the form loads.
        Required
        Set to Yes to make the field required, and No to keep the field optional.
        Order
        Specify a number to define the position of the field type on the form.
      • To add a Text, Date, Number, or Textarea field type, specify this information:
        Field Label
        Specify a name for the field. This is displayed on the form.
        Required
        Set to Yes to make the field required and No to make the field optional.
        Order
        Specify a number to define the position of the field type on the form.
      • To add a Checkbox or a Radio Button field type, specify this information:
        Field Label
        Specify a name for the field. This is displayed on the form.
        Option Name
        Specify the name of an existing check box or radio button option.
        Default Value
        Select the value that shows in the field when the form loads.
        Order
        Specify a number to define the position of the field type on the form.
    4. Click Save.
  9. To delete a field type from the form:
    1. In the Fields tab, select the field type from the list.
    2. Click Delete.
    3. Click Yes.
