Revising a pending name change request

Some fields are available only when they are configured by your organization.
  1. Select Profile > Personal.
  2. In the Personal Information pane, click Pending Name Changes.
  3. Double-click a row.
  4. Specify this information:
    Effective Date
    Specify when the change will take or took effect. If the change is a result of a court order, use the date of the document.
    Reason
    Specify the reason for the name change request. Optionally, click the Search button to select a predefined reason.
    Title
    Optionally, specify a title that is associated with the new name. For example, Ms.
    First Name
    Specify the new legal first name.
    Middle Name
    Specify the new legal middle name.
    Last Name
    Specify the new legal last name.
    Suffix
    Optionally, specify a suffix associated with the new name. For example, Jr or III.
    Professional Designation
    Optionally, specify a designation associated with the new name. For example, MD or PhD.
    Preferred Given Name
    Optionally, specify your preferred given name or what you would like people to address you.
    Preferred Last Name
    Optionally, specify your preferred last name.
    Marital Status
    Specify your marital status, if it has changed.
    Former First Name
    Optionally, specify your legal first name before the change.
    Former Middle Initial
    Optionally, specify your legal middle initial before the change.
    Former Last Name
    Optionally, specify your legal last name before the change.
    Attachment
    Optionally, specify a description of the attachment and attach the document that supports your request, such as a court order. If you skip this step and your organization requires approval of name changes, then you might be asked to upload an attachment later.
    Comments
    Optionally, specify a comment or other notes about your request for name change.
  5. Click Submit.
