Declining evidence of insurability requests You can either approve or decline Evidence of Insurability (EOI) requests for benefit plans. See Approving evidence of insurability requests. Select Maintenance > Evidence of Insurability. In the Pending section, select an employee EOI record and right-click. Click Decline EOI. Specify this information: Decline Date Specify the date of declining the EIO request. An error is displayed if you specify the Declined Amount and Decline Salary Multiple fields at the same time. Decline Amount You can decline an amount or a multiple. Declined Salary Multiple Specify an amount of the declined salary multiple. You must specify an amount that is within the limits of the benefit plan specified in the coverage rules. See Creating benefit plan coverage. Email Template Specify an email template from the templates previously created. Create New Benefit Select a benefit option if EIO is applicable. Click Submit. You can view the Status of the declined EOI in the All section. TipWhen declining an enrollment that does include the contribution amount subject to EOI, you can create a new benefit. The decline is not noted on this new enrollment and a new employee benefit record with the new contribution amount is created.
