This action is sometimes located within More Actions (ellipsis).
The Incident page is displayed.
On the Cause Assessment tab, click Create Incident Cause.
Complete the Description field with a concise name for this cause.
Complete the Category field by selecting Root, Direct, or Indirect Dotted Line.
Complete the Causal Factor field by selecting one classification that has been pre-configured within your organization. For example, you can choose Human Factors, Work Environment, or Procedures.
Complete the Summary field with your description of the reason that the incident occurred.
