You must report the cost of Employer Sponsored Health Coverage in Box 12 of your W-2 forms. See http://www.irs.gov.

The cost of Employer Sponsored Health Coverage prints with Code DD in Box 12 of the W-2.

Use the Code DD Employer Health Coverage Additional field located at Payroll Administrator > Regulatory Reporting > Year End Reporting > Employee Configurable Fields to specify an amount that will add to or subtract from the employee's year to date deduction history found from the deductions associated with the employer health coverage deduction class.

Use the Code DD Employer Health Coverage field to specify an amount to report in Box 12 Code DD. This amount overrides the employee's year to date deduction history found from the deductions associated with the employer health coverage deduction class. See Box 12 codes
