Defining appraisal forms
For information on how overall appraisal ratings are calculated and calculation examples, see Performance appraisal score calculation.
- Select Administrator > Performance > Appraisal Forms.
- On the Forms tab, click Create.

Specify this information:
- Form

Specify a short and long description for the form. The long description displays on the appraisal form.
- Eligibility Group

To restrict the form to one eligibility group, select the eligibility group.
Eligibility groups are custom groups defined in HR Talent that let you define a group of employees with specific characteristics. For Performance Management, eligibility groups must be defined with a business class of WorkAssignment and a subject of HCMGroupAppraisal. For example, if you use a different appraisal form for managers and non-managers, you could create an eligibility group for each.

- Form Total Weight

As you add and weight sections, this field keeps a running total of all the section weights. This field helps you monitor the total weight if the Must Equal 100% check box is selected on Organization Configuration. This selection means that your organization requires the total weight of all sections to be 100%.
- Active

The appraisal form is active by default. Clear the check box to inactivate it.
