Creating a safety observation (manager)
Perform this task to report an observation that involved more than one of your direct reports. You can also create observations for employees who are on a work assignment for which you are specified as Is A Supervisor.
- Sign in as Manager.
- Select Health and Safety > Create Safety Observation.

On the Create Safety Observation page, specify
this information:
- Short Description

A short description of what was observed.
- Observation Type

Select Safety Incident or Near Miss.
- Is This An Emergency Situation?
- Select the check box if the employees, others, or property was in immediate danger.
- Not Work Related
- Select this check box if the incident was unrelated to the jobs of the affected employees. Or, select this if an illness contracted by the employees was contracted outside of the workplace. If this check box is selected, and if this is a safety incident, the Include In OSHA Logs check box for the incident is unavailable, and the incident is excluded from OSHA reports. This feature prevents non-work-related incidents from being sent to OSHA for recordkeeping.

Specify the date reported.
- Observer

The default value is your employee ID.
- Observation Date and Time

Specify the date and time you observed the incident.
- Observation Time Undetermined

If you are unable to remember the exact time of the incident, select this check box. You can use another field to estimate the time or workshift.
- Estimated Time or Workshift

If you selected the Observation Time Undetermined check box, specify the approximate time or workshift in which the incident was observed.
- Location

Select the location where the safety or near miss incident occurred. If the Observer field has a value, the default value here is the employee's primary work assignment location.
- Not on Company Premises

Select this check box if the incident happened outside of the company premises.
- Location Details

Provide additional location information, if any.
- Select Impacted Employees
- Select the employees who were injured or harmed in this occurrence.
- Detailed Description
- Provide an account of what happened.

Click Submit to save the
information.
An observation record is created for each of the impacted employees on the Manager Safety Log page. A record is also created on Report Safety Observations page that each employee can access. The employee who was specified in the Observer field can access details of the observation.
Disregard the Schedule option, as most organizations require staff to report safety observations immediately.
