Creating COBRA participants manually

You can create two types of COBRA participants manually:
  • Terminated employee
  • Employee's dependent
  1. Select Benefits > Maintenance > COBRA Participants > By COBRA Participants.
  2. Click Create.
  3. Specify this information in the header:
    Employment ID
    Specify the employee ID number to create a COBRA participant that is a terminated employee.

    The address information is specified from the employee record. All the employee's current dependents and any identification numbers are created.

    Dependent
    Specify the dependent number to create a COBRA participant that is a dependent.

    An existing dependent number is related to the employee ID number.

    The address information is specified from the dependent record. All the employee's existing dependents are created. Any identification numbers for the dependent are created.

  4. Specify the information of the COBRA participant to create on the Main tab:
    1. Specify the COBRA participant's name information:
      • Title
      • First name
      • Middle name
      • Last name
      • Suffix
      • Professional designation
      • Preferred first name
      • Preferred last name
      • Gender
      • Marital status
    2. Specify the COBRA participant's personal information:
      Birthdate
      Specify the COBRA participant's date of birth.
      Date of death
      Specify the COBRA participant's date of death when applicable.
      Gender
      Select the COBRA participant's gender.

      Select Withhold to make the COBRA participant's gender undisclosed.

      Marital Status
      Select the current marital status of the COBRA participant.
      Smoker
      Select this check box if the COBRA participant to create is a smoker.

      Leave this check box blank if the COBRA participant is not a smoker.

      Consent
      Select the COBRA participant's permission in handling personal information.
      Medicare Indicator
      Select the coverage of the COBRA participant's health plan.
      Benefits Country
      Select the country where the COBRA participant's benefit is applicable.
      Currency
      Select the currency to use for the COBRA participant's benefits.
    3. Provide your comments.
  5. Click the Contact Information tab and specify the contact information of the COBRA participant to create:
    1. Specify the COBRA participant's contact information details:
      • Phone country or jurisdiction code
      • Phone number
      • Extension
      • Email address
      • Locale
    2. Specify the COBRA participant's address details:
      Country/Jurisdiction
      Select the country or jurisdiction of the COBRA participant's address then specify the COBRA participant's full address to search automatically.
      Enter Address Manually
      Select this check box to specify manually the street and city of the COBRA participant's address.
      Search For Address
      Select this check box to search the COBRA participant's full address automatically.

      If you select the Enter Address Manually check box, then this check box is displayed.

  6. Click Save.
    On the Identification Numbers tab, the descriptions should match the descriptions in the employee documentation.
