Disability self-identification

Depending upon the timing of the release, the new document template might be automatically uploaded and available in the system. In that case, no action is required. If the template has not been uploaded automatically, then you must download the PDF from this topic. Then you must manually upload the PDF to the system and associate it with a template.

Every few years, the US Government releases a new Disability Self-Identification Form and associated PDF. Because the Disability Self-Identification Transition Form uses top and bottom instructions, if the form is changed, the instructions must also be updated.
