Reporting a safety or near-miss incident

You must have the Safety Administrator or Facility Manager role to report an incident that you were not personally involved in. That is, you can make a report on behalf of an employee.
  1. Sign in as Health & Safety Management.
  2. Select New Incident > Enter A Safety Incident.
    Or
    Select New Incident > Enter A Near Miss.
  3. Provide information as you would when viewing or updating a safety-incident record.

    If this is an incident for inclusion in state or federal reports, select Include In OSHA Logs on the Details tab of the incident record.

  4. Select Save.
    Optionally, view the new safety incident by selecting Safety Management > Safety Incidents.

    View the new near miss by selecting Safety Management > Near Misses.
