OSHA Form 300 log

Log of Work-Related Injuries and Illnesses is used to classify work-related injuries and illnesses and to note the extent and severity of each case. When an incident occurs, use the log to record specific details about what happened and how it happened.

OSHA Form 300A summary

Summary of Work-Related Injuries and Illnesses shows the totals from OSHA 300 in each category for the year. The report must be completed and posted each year even if no work-related injuries or illnesses occurred during the year.

OSHA Form 301 report

Injury and Illness Incident Report contains details about each incident. The report must be completed within seven calendar days after you receive information that a recordable work-related injury or illness has occurred. This report contains employee-specific information, treatment that was given, date of injury, and a description of what happened.
