Generating an incident from an observation

Perform this task if an observation must be treated as an incident.

  1. Sign in as Health & Safety Management.
  2. Select Observation Log.
  3. On the New tab or In Review tab, select a record.
  4. Click the Generate Incident action.
  5. Optionally, edit this incident information, which was carried over from the observation record:
    Description
    Provide a brief summary of what happened.
    Incident Date, Incident Time
    Select the date and if possible, the time when the incident occurred.
    Incident Time Undetermined
    Select this check box if the observer is not sure of the time.
    Location
    Select from among the possible incident locations that are configured by the organization. If there is not an accurate location available, then you can create one from the selection page by clicking Create.
    Incident Type
    This value is copied from the observation that is the basis for generating the incident. If the incident is generated from a COVID-19 observation, then the default value of this field is COVID-19 Incident. You can change the value. If no value is suitable, then you can add one.     Not Work Related
    Select this check box if the incident was unrelated to the job of the employee. Or, select this if an illness contracted by the employee was contracted outside of the workplace. If this check box is selected, then the Include In OSHA Logs check box for the incident is unavailable, and this incident is excluded from OSHA reports. This feature prevents non-work-related incidents from being sent to OSHA for recordkeeping.
    Include in OSHA Logs
    This check box is available for work-related safety incidents only. Select this check box to add the incident to OSHA logs. If you leave the check box blank, then the incident will be omitted from OSHA reporting. Omit this from reports only if it is a duplicate or non-qualifying incident according to your organization's policy and the safety administrator's discretion. For example, a small cut might not qualify for OSHA reporting, while a needlestick injury must be reported to OSHA.
    Safety Category
    This category, if any, is copied from the observation record. Add or modify the category selection for this incident for use in reporting and lookup.

  6. Click Submit.
    The incident is created, and a details page is displayed, where you can provide or edit additional information such as impacted people, causes, and costs.

    The observation is moved to the Active tab of the Observation Log. It is also added to the Observations tab of the incident record that you created. You can find the new incident by selecting Safety Management > Safety Incidents or Safety Management > Near Misses.
