Creating eligibility surveys

An eligibility survey is a series of questions asked of an employee to determine the eligibility of the spouse or domestic partner for identified benefits. The survey can contain up to five questions in a "Yes or No" answer format to determine eligibility.

  1. Select Benefits > Setup > Eligibility Surveys.
  2. Click Actions > Create.
  3. Specify this information:
    Eligibility Survey
    Survey code.
    Description
    The text that is shown to the employee within enrollment as the name of the survey.
    Start Date
    The version of the survey. When a survey is included in enrollment, the most recent version of the survey is displayed for the employee whose start date that is less than or equal to the enrollment date and that is active.
    Relationships Types Covered
    Identify the relationship types to which the survey applies.
    Question 1-5
    Specify a question and eligibility results for Yes and No answers. The results identify the eligibility or if the employee can continue to the next question.
    Eligible
    The employee’s dependent with the relationship type is eligible for benefits that include a survey. There is no additional contribution.
    Eligible With Surcharge
    The employee’s dependent with the relationship type is eligible for benefits that include a survey. There is an additional contribution that is applied. The additional contribution is defined on the benefit plan’s contribution rules.
    Not Eligible
    The employee’s dependent with the relationship type is not eligible for benefits that include a survey.
    Not Applicable
    The employee’s dependent with the relationship type is not eligible for benefits that include a survey. This is the same as Not Eligible. It is provided because the employee can see this result and in some cases “Not Applicable” makes more sense than “Not Eligible”. For example, when a question is “Do you have a spouse?”
    Continue
    When the employee selects a response with this result then the next question is presented. You cannot select this response for Question 5 or for the last question if the surveys has less than five questions.
