Time Off Requests form

Use this form to create or withdraw time-off requests, along with a calendar to view the requests for the current and previous year.

The form displays:
  • Operator name
  • List of time off requests submitted by the operator for the current and previous year.
  • Status of the request.
  • Cancel option to cancel the request if the time-off is not required by the operator.
