Setup for requests for testing

This setup information is provided in Optiva.

• A default workflow action to control the creation of the sample and test order and the opening of the Test Results form.
• A default create rule to create the samples and test orders. This is used by the workflow.
• A default profile attribute that is used to specify the workflow to run and what information is displayed.
