Expiration Date
On the bottom of the form, enter the date the effectiveness of the second document used expires.
The employee must provide as documentation proof:
one item from List A on Form I-9
OR
one item from List B AND one item from List C.
Note: The Document 2 fields are not accessed if the Document 1 I-9 List field shows the required proof of work eligibility and identity included the one item required from List A.
