Adding header options to Survey

Use the Add Header to survey window for adding header information to a survey.

  1. Select System Administration > Survey. The Survey list page is displayed.
  2. Select the required survey. The Survey details page is displayed.
  3. Click Insert in the header section. The Add Survey Header Option window is displayed.
  4. Specify this information:
    Survey Header Option
    The name of the survey header.
    Sequence
    The sequence in which the header options are displayed.
    Question Type
    The format in which the survey question is displayed. Possible values:
    • Date/Time
    • Text
    • Toggle
    • Currency
    • Integer
    • Decimal
    • Percentage
    • Dropdown
    Attributes
    The option format that is provided for the question type. Click Create Attributes to specify the Attributes.
  5. Click the Create Attributes option to specify the attributes in the Create Survey Attributes window displayed.
  6. Select the Required check box to receive an answer for a survey question when executing a survey.
  7. Click Submit.
