Adding header options to Survey
Use the Add Header to survey window for adding header information to a survey.
- Select System Administration > Survey. The Survey list page is displayed.
- Select the required survey. The Survey details page is displayed.
- Click Insert in the header section. The Add Survey Header Option window is displayed.

Specify this information:
- Survey Header Option
- The name of the survey header.
- Sequence
- The sequence in which the header options are displayed.
- Question Type
- The format in which the survey question is displayed.
Possible values:
- Date/Time
- Text
- Toggle
- Currency
- Integer
- Decimal
- Percentage
- Dropdown
- Attributes
- The option format that is provided for the question type. Click Create Attributes to specify the Attributes.
- Click the Create Attributes option to specify the attributes in the Create Survey Attributes window displayed.
- Select the Required check box to receive an answer for a survey question when executing a survey.
- Click Submit.
